Just went to the restroom, was on her way back to bed, unassisted when she fell.

It was. An unwitnessed fall.

the nursing staff heard her fall, went right to her side.

She said that her legs just gave out.

She's complaining of right-sided muscular neck pain and an injury to the back of her right hand.
